What does the Data Team do?
The Data team assists schools and other organizations with immunization data entry by entering historical records into CHIRP.

How do I sign up for data entry help?
To begin the process, fill out the form by clicking the button above. A Data Team member will contact you to schedule a date and time.

Can the Data Team come to my organization?
Yes, we can! We will schedule a date and time that fits your schedule at your location.

Can you enter our records at your location?
No. We do not take any records with us. All of the data entry must be done at your location.

What do I need to do to prepare for the Data Team’s visit?
Please have your historical immunization records ready for review, a place for us to work with access to electrical outlets, and a secure wifi connection available. Please ensure signed FERPAs are up to date before our arrival. You are responsible for gathering this data.

Do I have to be present while the Data Team works?
Yes. While you are not required to assist us with the data entry, we ask that you be available if any questions arise.

What will the Data Team do?
The Data Team will enter the historical immunization records into CHIRP and train your team members on best practices for entering future records and running reports. The Team will be available to answer any questions and address concerns. They will also provide resources and helpful information to staff to have on file for future reference.

How long will the data entry project take?
We have a team of four data entry specialists who will assist you. The time needed to enter the data will depend on the amount of records you have. However, the project will be completed faster if the CHIRP profiles have already been created in the system.

Can the Data Team create a patient profile?
If a patient profile in CHIRP is non-existent, the Data Team can create one. To do so, we will need the following information:

  • First and last name
  • Date of birth
  • Gender
  • Race
  • Patient’s address
  • Parent/Guardian name
  • Phone Number

Are there any records you can’t enter?
We will not enter historical immunization records for students who do not have a signed FERPA consent by a parent or guardian on file.

What if I have immunization records in languages other than English?
Email [email protected], and we can assist you with this.

Can you show me how to run reports in CHIRP?
Yes. While we are onsite, we will show you how to run reports. We can also email you a training manual. To request one, email [email protected].
